Life Changing Care is a home care agency based in Cleveland, Ohio, serving families across Cuyahoga County. We provide non-medical home care services including personal care, companion care, dementia care, and Direct Service Provider support.

Our website is provided for informational purposes and to allow families to request care services. You agree to use this website only for lawful purposes and in a way that does not infringe on the rights of others.

You agree not to:

• Submit false or misleading information
• Attempt to gain unauthorized access to our systems
• Use our website to transmit spam or harmful content

The information on this website describes the types of home care services we offer. Actual services are provided under a separate care agreement between Life Changing Care and the client or their authorized representative. Nothing on this website constitutes a contract for care services.

We reserve the right to decline to provide services at our discretion.

Life Changing Care operates two text messaging programs. By opting in on our contact form, you agree to the following terms for each program you enroll in.

Program 1 — Care coordination:

Life Changing Care sends text messages related to care coordination, consultation scheduling, appointment reminders, intake confirmations, and account notifications.

Program 2 — Service updates:

Life Changing Care sends text messages about home care resources, caregiver availability, and general service information.

Applies to both programs:

• Consent is optional. Opting in to text messages is not required to receive home care services from Life Changing Care.
• Message frequency varies based on your care needs and activity.
• Message and data rates may apply depending on your mobile carrier and plan.
• To cancel: Reply STOP to any text message at any time. You will receive one final confirmation message and then no further messages from that program.
• To re-subscribe: Text START to the number you received messages from.

• Carrier disclaimer: Mobile carriers are not liable for delayed or undelivered messages.
